January 2008

The following PNAMH past presidents initiated the formation of PNAMH Foundation:

May 03, 2008 

​

The Constitution and By-Laws were finalized/ notarized and sent to the Secretary of State in Austin, Texas with the following officers: 

  • Chairperson - Cleo M. Robinett Vice Chairperson - Eufe S. Chua 

  • Secretary - Grace Diaz - Font Treasurer 

  • Gloria L. Beriones 

  • Board of Trustee - Harley Tuble

July 28, 2008 

​

Certificate of Approval as a Non-Profit Corporation from the Texas Secretary of State was received. 

​

April 30, 2009 

​

Preparation of the application for Recognition of Exemption under Section 501 (3) of the 

Internal Revenue Code was initiated 

The initial funding (seed money) of $ 1000 was donated by Harley Tuble to form the PNAMH Foundation. The following documentation was prepared and submitted to the Internal Revenue Service (IRS) with the application.

Scholarship Program - prepared by Grace Diaz - Font 

Fund Raising Programs such as Musical Extravaganza, Fun Walk; and "Friends "of PNAMH - prepared by Cleo Robinett 

Educational Calendar- prepared by Cherry Sloan-Medrano 

Required Form 1023 Financial Data for four (4) years from 7/01/2008 - 12/31/2011 prepared by Vic J. Chua, CPA 

The process of application was coordinated, completed, and submitted by Eufe S. Chua. 

 

July 14, 2009

A determination letter from the IRS was received, indicating PNAMHFI was approved as an exempt organization under the nonprofit corporation code of charitable and community programs, Scholarship, Educational, Scientific, and Research.

August 2012 

​

The constitution and by-laws were amended to change the Officer title from Chairperson to President and the Vice Chairperson to Vice President. 

Current activities/funding supporting PNAMH includes but are not limited to Annual Scholarships, Educational programs such as monthly educational offerings, Spring and Fall Seminars, PNAA Regional and National Conferences, Opioid Addiction with emphasis on Awareness and Education to High School students, Charitable and Humanitarian Services and other PNAMH programs, ie Balik Tulong.
